• Mail Handler Benefit Plan (For Federal employees)

• Medicare

We are not contracted with the following insurance:

• AHCCCS
• All Blue Cross Blue Shield Except for the above mentioned
• United Health Care
• Tricare
• Multiplan
• Great West

Billing to Insurance companies that we are not contracted with:

As a courtesy, we submit these claims to the appropriate insurers for out-of-network coverage except for Medicare and AHCCCS.
Each insurer then reimburses you, the patient, according to its plans and policies. Responsibility for coverage determination and collection remains with you, the patient.

We apologize for not being able to bill AHCCCS for you.

If we are not contracted with your insurance as listed above, you have the option of paying with cash or credit card at the time of your visit.
